Personal Information Consent Forms

We are very excited to share that thanks to our leadership students JL Jackson will have a yearbook this year! Unfortunately, for student photos to appear in the yearbook both the Student Personal Information Consent Form and Parent Personal Information Consent Form below need to be signed for each student.

Block A teachers have a list of the forms that have been completed for each student and will be reminding them to get their forms signed if they want to be included in the yearbook. At the moment we have almost 200 students who will not be featured in the yearbook unless these forms are signed! Thank you for helping to ensure this task is completed at your earliest convenience.

Paper copies are available in the office or you can print and sign the forms below.

Establishing safe, caring & respectful digital communities

Would you like to help your youth in navigating challenging topics such as cyberbullying, sexting and sextortion?

A virtual Social Media Awareness Family Session is being held by Safer Schools Together and the Ministry of Education and Childcare. This session is designed to support families and caregivers to navigate the digital world and there are four dates to choose from!

This one-hour session covers all things digital, from how to be a good digital citizen, the criminal consequences of cyberbullying, and how to report and receive support in cases of sexting/sextortion. The session is recommended for Parents/Caregivers/Grandparents and youth aged 10 and up as a way to facilitate the conversation of safe and caring use of technology at home and in the community.

Some of the most prominent concerns currently surrounding use of social media platforms, smartphones, and artificial intelligence will be discussed.

Families will learn:

  • Digital media habits, oversharing, and digital addiction
  • The permanence of online posts/activity
  • The significance of your digital tattoo in how others perceive you (e.g., sports teams, college admissions, and potential employers)
  • The importance of caring for peers both online and offline
  • Geo-locational settings and the dangers of social media and cell phone misuse
  • The social and legal consequences of cyberbullying behaviour as well as possession and distribution of intimate images (sexting and sextortion)
  • An anonymous student reporting tool that can help you and your friends stay safe both online and offline

Upcoming Training for Families with children/youth 10 & up will take place:
