Personal Information Consent Forms

We are very excited to share that thanks to our leadership students JL Jackson will have a yearbook this year! Unfortunately, for student photos to appear in the yearbook both the Student Personal Information Consent Form and Parent Personal Information Consent Form below need to be signed for each student.

Block A teachers have a list of the forms that have been completed for each student and will be reminding them to get their forms signed if they want to be included in the yearbook. At the moment we have almost 200 students who will not be featured in the yearbook unless these forms are signed! Thank you for helping to ensure this task is completed at your earliest convenience.

Paper copies are available in the office or you can print and sign the forms below.
